About Dr. Nirvana Swamy Kudlur Chandrappa
Dr. Chandrappa is a Consultant Psychiatrist with UK specialist training and consultant experience in NHS mental-health services and internationally.
After qualifying in medicine at the University of Mysore, he completed postgraduate psychiatric training in the UK, becoming a Member of the Royal College of Psychiatrists (MRCPsych) and receiving a Certificate of Completion of Specialist Training (CCST) in General Adult Psychiatry and Substance Misuse (Addiction) Psychiatry.
As a consultant with Worcestershire Health & Care NHS Trust he worked across inpatient and community services, led the Trust's Electroconvulsive Therapy (ECT) service, and held Clinical Director roles in acute mental health and in substance misuse and prison mental-health services. He later worked as a Senior Consultant Psychiatrist in Consultation Liaison Psychiatry at Hamad Medical Corporation in Qatar, where he contributed to published research on addiction, mental-health services and healthcare-worker wellbeing.

What Does a Consultant Psychiatrist Do?
A consultant psychiatrist is a medically qualified doctor who has completed specialist training in mental health. Because psychiatrists are doctors, they can consider how physical health, medication and mental health interact — and can prescribe treatment where appropriate.

rTMS & ECT, explained simply
Neuromodulation treatments influence brain activity using magnetic or electrical stimulation. They are considered for specific conditions only after a full psychiatric assessment, and are not suitable for everyone. No treatment can guarantee a particular outcome.

Repetitive Transcranial Magnetic Stimulation
rTMS uses a magnetic coil placed against the scalp to deliver pulses that stimulate specific areas of the brain involved in mood regulation. It does not require anaesthesia, and people are usually awake and able to resume normal activities after each session.
In the UK, rTMS is most commonly considered for depression, particularly when other treatments have not helped enough. A course typically involves regular sessions over several weeks. Like all treatments, it has potential side effects — such as headache or scalp discomfort — and is not suitable for everyone.
ECT
Electroconvulsive Therapy
ECT is carried out under general anaesthetic and muscle relaxant. A brief, controlled electrical current is passed through the brain to produce a short seizure. It is delivered in specialist clinical settings by a trained team.
ECT may be considered for severe depression and certain other serious conditions, usually when other treatments have not been effective or when a rapid response is clinically needed. Potential side effects, including effects on memory, are discussed in detail as part of informed consent.
Dr. Chandrappa previously led an NHS ECT service. Where ECT may be relevant, a consultation can help consider whether a referral to an appropriate service is suitable.

What happens during an initial psychiatric consultation?
Dr. Chandrappa will ask about your current concerns and symptoms, your mental and physical health history, any previous treatment and medication, and your personal circumstances and goals. At the end, he will share his initial impressions and discuss possible next steps. You are welcome to ask questions throughout.
How long is a consultation?
Appointment lengths vary depending on the type of consultation. Initial assessments are longer than follow-up appointments. The practice will confirm the length of your appointment when it responds to your enquiry.
What conditions can you assess?
Dr. Chandrappa is a general adult psychiatrist with additional specialist training in addiction psychiatry. Clinical interests include mood and anxiety disorders, OCD, adult ADHD, autism-related mental-health needs, addiction, psychosis, personality disorders, neuropsychiatry and neuromodulation. Do you assess adult ADHD?
Yes, adult ADHD is one of Dr. Chandrappa's clinical interests. An assessment explores current and childhood difficulties and considers other possible explanations. An assessment does not guarantee a diagnosis or a particular treatment.
